Unless you have a really good connection with your opponent 5 bars, escaping grabs is very hard/impossible on reaction online. The only thing you can do is predict when the opponent will grab duck and punish with you best WS move. However offline you have to see wich arms they use when they grab you, 1+3 grabs(left arm) you hit the 1 button, 2+4 grabs(right arm) you hit the 2 button. If they grab you with both arms you have to press 1+2.
You can practice grab breaking in practice, select defensive mode and go to command list. I would start with learning to see the difference between 1+3, 2+4 throws, then after a while you can add grabs with 1+2 escape.
If you training punishment against a character, why not add some throws in the mix. This way wont know if a throw is coming. It is also a good idea to train throw breaks against different characters.
Its going to take a while before you get good at it though, you will need a lot of patience. Practice throw breaking for 1 hour at a time, do it often. And after some time maybe 2 weeks you should at least see some results in matches like 15-20% of 1+3,2+4 throws broken, 1+2 breaks takes longer to learn in my experience.
